My National Guard unit is being activated under Title 10 or Title 32. Will my Technician Disability Insurance cover me during my tour of duty?
It depends on the length of your tour. Yes, periods of duty less than 30 days will be covered under the disability plan. On the other hand, your TechPay Disability Insurance will not be in effect after you enter active duty (including active duty for training) in federal service under orders that specify a period of duty of over 30 days, or service in the armed forces of a foreign or international authority. ReliaStar will waive premiums for the initial 30 days. After 30 days, you will be covered under federal law and your policy will be put into a "suspense account." No premiums will be due until you return from active duty. At that point, we will reinstate you and there will be no lapse in coverage. Technicians do not have to "make up" back premiums for the 30 day premium waiver when payroll deductions resume. However, you must first complete the NG76 form with your human resources officer so that both the Insurance Trust and ReliaStar will know that you are in a leave without pay status. Also, when you return to your technician duties you must notify ReliaStar within 30 days that your leave without pay status has ended.
How do technicians pay premiums for TechLife Insurance while on leave without pay status?
ReliaStar will waive premiums for 90 days. If you are not back from active duty within 90 days then you must pay your premiums through direct bill. Please provide a mailing address to ReliaStar while you are on leave without pay. If there is a death claim during the 90-day waiver period, ReliaStar will honor the policy. There will be no lapse in coverage and you are not required to "make up" back premiums.
I have Universal Life (Horizon Plus Product) Insurance coverage, what do I need to do to keep the coverage while I am on leave without pay?
There is no waiver of premium with this product. ReliaStar will direct bill from the time that you go on leave without pay. This is necessary due to the investment component of this product. If you have an Accidental Death Rider, your beneficiary or beneficiaries cannot collect if you are killed while engaging in "acts of war."
I have Basic/Supplemental Disability Insurance. How do I file a disability claim?
To file a disability claim, you should click here for filing instructions and claim forms. Please discontinue using form #23885b (Disability Notice Claim). There are several forms to complete.The Human Resources Officer (HRO) and or Technician Personnel Officer completes the 1) Employers Statement and the 2) Occupational Demands forms. The employee receives the completed "Occupational Demands" form with a copy of the employee's job description attached and the following forms:
3) Long Term Disability Employee's Statement,
4) Attending Physician's Statement of Impairment and Function,
5) Authorization for Release of Health-Related Information, and
6) Consumer Privacy Notice
All of the completed forms should be returned to the HRO for submission to ReliaStar Life. It is very important that the Plan Number, contract Number and Group Number is filled in on the claim form: GL, H 12454-1. How do I change my beneficiary or beneficiaries for my Insurance Trust coverage?
You need to complete form #7384 (Request for Change of Beneficiary). Please keep your beneficiary's designations up-to-date especially after events such as marriage, divorce, birth, name change and death. Also, you can name a contingent beneficiary or beneficiaries on a separate sheet of paper. If you need more space, to write all of your beneficiaries' names, you can list them on an additional sheet of paper. Make sure that you write their names and plan/policy number on a separate sheet of paper. Sign and date this paper and attach it to your enrollment form. You may name a trust, charity, organization or your estate as beneficiary on your enrollment form. An irrevocable beneficiary is one whose interest in the policy cannot be changed or reduced without his/her consent.
I recently gave birth to an additional child and I have a Children's Rider on my policy. What do I need to do to make sure that my additional children are covered?
First of all, they are automatically covered under your policy. You may elect to send in their names and date of birth to ReliaStar so that they can add the information to their files.
I lost or misplaced my insurance certificate; how do I get a new one?
Call either the Insurance Trust at (888) 642-8748 or ReliaStar at (800) 955-7736 Ext. 352-3155 to request a duplicate to be sent directly to your mailing address.
How do I find out when new benefits/services are added?
You should visit our website,www.ngaus.org at least once a week to see the latest benefits information. In addition, you can call the Insurance Trust Member Benefits office at (888) 642-8748 between the hours of 8AM - 5PM Monday - Friday, Eastern Standard Time.
